Has anyone tried this? I looked at the code and it is all confusticated in an eval(). Why would I trust this?

The eval is part of the compression to keep the file size minimum.

If you are mart enough to read the code and evaluate its safety, then you are smart enough to decompress it.

Also, what do you need to trust? It is a bookmarklet. The damage it could conceivably do is extremely minimal. If you want to be safe, the only safety measure you need to take is to log out of YouTube before using the bookmarklet and make sure that you refresh the page before you login again.
